Please use this form to request funding for an activity or sport that is supported by the ERA Booster Club.

Funding requests will be reviewed during three meetings throughout the school year.  Please make sure you get your funding requests in by one of the following three deadlines

• September 15 (for approval during September meeting)
• January 15 (for approval during January meeting)
• March 15 (for approval during March meeting)

*This form is ONLY for requesting funds for a FUTURE need.

If you’ve already made a purchase and need reimbursement, please use the BoosterHub app:

• Go to Tools in the bottom navigation.
• Select Check Request.
• Upload the details along with a copy of your receipt or invoice.
